How does the assessment work?
If you are eligible, you may receive a letter from either Signify Health or Matrix Medical Network to schedule your visit. The letter will contain a number you can call to schedule your appointment. The licensed medical professionals working for Signify Health or Matrix conduct the assessment in the comfort of your own home. It includes taking basic vitals, such as your blood pressure and pulse. During the assessment, the medical professional conducting the assessment may:
  • Review recommended screenings based on your age and health status
  • Discuss relevant information on health conditions, medicines and preventive care
  • Answer your questions about the medicines you take and the best way to take them
To get the most out of your assessment, we encourage you to schedule a follow-up with your Primary Care Provider (PCP) to go over the results. If you do not have a PCP, Humana can help you find one in the Humana network. You can also find a PCP through Humana’s Find a doctor tool.
Is the assessment required?
No, the assessment is optional. However, it is a great opportunity to get up to an hour of dedicated and personalized time focused solely on you and your health. The assessment is beneficial to both you and your doctor as it gives an extra pair of eyes and ears to your personal health goals.
What are the assessment results used for?
You will receive a brief report that will highlight suggested topics to discuss with your doctor at your next visit. If you have a PCP, Humana will also share the assessment results with them. This will help you and your doctor manage your overall health. The assessment is not a substitute for regular doctor visits, but it does help to provide another layer of support for you. Based on your results, Humana may also suggest resources that can further assist you with your health goals. The results do not affect your health plan rates or benefits.*
How long does the assessment take?
Your assessment will take about 45 minutes to an hour.
Can family members be present during the assessment?
Yes, you are welcome to have anyone you think is appropriate present during the assessment. Especially if this person helps to manage your care, it may be helpful for them to be able to speak with the licensed medical professional and discuss your health goals.
Can I receive more than one Health and Well-being Assessment a year?
Humana only provides one Health and Well-being Assessment per calendar year.
How much does the assessment cost?
Humana offers the service to members as part of their plan at no additional cost.
Does the Health and Well-being Assessment take the place of my Annual Wellness Visit with my doctor?
No, the Health and Well-being Assessment doesn't take the place of your Annual Wellness Visit. It is an additional preventive care visit.
Does this program replace my regular doctor visits?
The Health and Well-being Assessment doesn’t replace your regular doctor visits, or any other part of your regular care. The assessment is designed to compliment the regular care you receive from your doctor.
Does this program replace my current home healthcare or care provided in a Humana Clinical Program?
No, this program doesn't replace any current home services or care provided by a Humana Clinical Program.
Will I get a copy of my assessment results?
Yes, you'll receive a summary of your results after your visit is completeIf you complete your visit virtually, you will receive a summary in the mail in about two to four weeks.  If you need another copy of your results or you did not receive them after your visit, you can call the number on the back of your Humana member ID card for assistance. 
Does the Health and Well-being Assessment offer any rewards for Go365®?
No, the visit is not part of Go365®. If you have access to Go365® through your Humana plan, the visit doesn't offer any points or other rewards for Go365®.
Why did I receive a call from Signify Health asking about scheduling a visit with my doctor?
One of the most important things you can do after your Health and Well-being Visit is to review the results with your doctor. Signify Health may call you after your visit to assist with scheduling a visit with your doctor. If you don’t have a doctor or you are interested in finding a new one, they may be able to assist you with that as well. If you do not have a PCP, you can call the number on the back of your Humana member ID card. A Humana customer care representative can help you find a PCP in your network. You can also find a PCP through Humana’s Find a doctor tool.
Can the medical professional that performed the health assessment become my PCP?
No, you will continue to see your current PCP for regular checkups, your Annual Wellness Visit and any additional ongoing care. If you do not have a PCP, you can call the number on the back of your Humana member ID card. A Humana customer care representative can help you find a PCP in your network. You can also find a PCP through Humana’s Find a doctor tool.
What are the qualifications of the medical professionals who perform the Health and Well-being Assessment?
The medical professionals from Signify Health and Matrix Medical Network have valid licenses to practice medicine. Just as Humana carefully screens our own providers, the medical professionals from Signify Health and Matrix must meet the same qualifications and expectations. All clinicians must be credentialed prior to working with Humana. This requirement is verified by Humana’s own credentialing team, and clinicians must be recredentialed every three years. The vendor providers also receive training on health assessments to maximize the usefulness of the assessment to your personal health requirements.
Do I need to remove my clothing?
No, you will not be required to remove any clothing for the assessment. Certain tests may require you to roll up your sleeve or remove your socks, but you will remain clothed for the duration of the assessment.
Does my PCP receive a copy of the results?
Yes, if you have a PCP, they will also receive a copy.
What should I discuss with the medical professional that performs my Health and Well-being Assessment?
The Health and Well-being Assessment offers time for you to discuss the details of your current and future health.    If you already see multiple doctors, this can also provide a good opportunity to discuss the care you receive from each doctor. This can give the medical professional chance to help you understand how different treatments may fit together and may provide you with insight to your overall health. 
How was I assigned to Matrix Medical Network or Signify Health?
Members are assigned to our vendors based on their location. Certain group plans may also be contracted under a certain vendor.
