Humana partners with Signify Health to bring professional health care directly to you. Signify Health’s medical professionals will provide you with personalized attention during your Health and Well-being Assessment, helping you get answers to your question and supporting your health goals.
Signify Health envisions a more connected, effective care experience for all. They work as another layer of support on your journey to better health, alongside Humana and your doctor.
Signify Health uses qualified and carefully screened medical professionals to perform well-being visits on behalf of Humana. These providers may include physicians (MD/DO), nurse practitioners and physician assistants. Signify Health providers meet all the standards required from a Humana provider.
A Signify Health medical professional will meet with you for up to an hour in the comfort and convenience of your home. You’ll receive personalized attention that may provide you with more insight into your health and further support the care you receive from your doctor.
Signify Health recommends their medical professionals and Humana members follow current health and safety precautions. If either of you are experiencing symptoms of illness at the time of the visit, we may cancel the appointment for health and safety reasons.
During your visit, you will receive:
A thorough Health and Well-being Assessment
A full review of your medical history and medications
Personalized recommendations for additional care, screenings and resources if needed
While we highly recommend in-person visits for all of our members, virtual options may also be also available.
If you’re unable to have visitors or prefer a virtual visit, Signify Health medical professionals may be able to provide many of the same benefits through a video assessment. All you need is a device with a camera and internet access.
